    A Brief Study on the Medical Science of Tamil Nadu from Ancient times

    (1) Medical Practitioners during Sangam Period

    In the ancient Tamil Nadu the "Medical Practitioners" were referred to as the "Maruththuvar" or as "Vaiththiyar" and the practice of Medicine as "Maruththuvam" or "Vaiththiyam".

    In "Tamil Saivaism" that developed in Tamil Nadu blended with the Culture and Traditions of "Ancient Tamil Nadu", the "God Siva" (God-as-Siva) is said to cure Human Beings from their accumulated illness of "Vinaikal" (Karma), like how the 'Medical Practitioners the "Maruththuvar" or "Vaiththiyar" cured their physical and phychological illnesses, and hence was 'also' given the names as "Marunthueeswarer" and "Vaiththiyanather".

    The earliest reference to Medical Practitioners (Doctors) of the third Thamil Sangam period is gleaned from Sangam Period Poetic work "Ahanaanooru" from which we note there had been a Poet of Third Thamil Sangam, whose actual profession was the practice of Medicine. He was known as "Uraiyiur Maruththuvar Thaamotharanaar". "Maruththuvan" was the actual term used by ancient Tamils of Tamil Nadu for a "Medical Practitioner". The Uraiyur prefixed to his name was the reference to his native place being the ancient capital city of Chola Nadu.

    Another Third Sangam period literary composition known as "Paripaadal" had the music composed for it's sixth Paadal on river "Vaikai" by another Medical Practitioner known as "Maruththuvar Nalachchuthanaar" who was also an expert in music. Urai Asiriyar - "Adiyaarkku Nallaar" in his commentry to 'Silappathikaaram' refers to another Medical Practitioner of third Thamil Sangam period as "Uruththirasanmar Maruththuvanaar".

    The Sage/Poet Thiruvalluvar of third Thamil Sangam of the first century B.C. Tamil Nadu, in a chapter titled "Marunthu" (Medicine) in his great treatise "Thirukkural" defines the "basics" of the Medical Practice prevalent during his period. It states the cause of "natural illness" in people was due to excessive food and less water intake, less excercise, variation in environmental climatic conditions, and improper function of body organs - abnormally at young age and in the old age, leaving out the "accidential illness" - due to external harm done by human beings, animal, birds, reptiles and insects, and internal harm done by poison and spoilt food.

    The Sage/Poet Thiruvalluvar in his Tamil Code of Ethics the "Thirukkural" in the chapter titled மருந்து (Medicine) clearly indicated the necessity of the control of the intake of Food in the form of solids and liquids.

    (2) Control of Food intake (solids and liquids) key to healthy life

    மருந்தென வேண்டாவாம் யாக்கைக்கு அருந்தியது
    அற்றது போற்றி உணின். 942

    அற்றால் அறவறிந்து உண்க அஃதுடம்பு
    பெற்றான் நெடிதுய்க்கும் ஆறு. 943

    அற்றது அறிந்து கடைப்பிடித்து மாறல்ல
    துய்க்க துவரப் பசித்து. 944

    மாறுபாடு இல்லாத உண்டி மறுத்துண்ணின்
    ஊறுபாடு இல்லை உயிர்க்கு. 945

    இழிவறிந்து உண்பான்கண் இன்பம்போல் நிற்கும்
    கழிபேர் இரையான்கண் நோய். 946

    தீயள வன்றித் தெரியான் பெரிதுண்ணின்

    (3) Medicines are essential to prevention and cure Illness

    The Tamil Sage Thirumoolar spells out the importance of Medicine for Human Beings as follows:

    One that cures physical ailment is medicine
    One that cures psychological ailment is medicine
    One that prevents ailment is medicine
    One that bestows immortality is medicine“

    (4) Practice of Medical Science during Sangam period

    (1) Practice of Medical Science - comprises of the Patient, the Medical Practitioner (Doctor), Medicines that cures the illness, and the Nursing Aides.

    உற்றவன் தீர்ப்பான் மருந்துழைச் செல்வானென்று
    அப்பால் நாற் கூற்றே மருந்து. 950

    (2) The Medical Practitioner (Doctor) in his practice of the Medical Science - should diagonise the type of Patient’s illness, the root cause of his illness, and analyse the method of curing the illness.

    நோய்நாடி நோய்முதல் நாடி அதுதணிக்கும்
    வாய்நாடி வாய்ப்பச் செயல். 948

    (3) The Medical Practitioner (Doctor) in his practice of the Medical Science - at the time of diagnosis give thought to the age of the Patient, the extent of his illness, and the period of illness.

    உற்றான் அளவும் பிணியளவும் காலமும்
    கற்றான் கருதிச் செயல். 949

    (4) The People who take much more or much less food than their normal intake, will be subject to three root causes of illness the Vali (Sanskrit - Vaatham), Thee (Sanskrit - Piththam), Neer (Sanskrit - Kapam) as indicated in the Medical Treatises.

    மிகினும் குறையினும் நோய் செய்யும் நூலோர்
    வளி முதலா எண்ணிய மூன்று. 941

    From the first Kural under the title "Marunthu" we note there had been Medical Treatises during the period of Thiruvalluvar which is gleaned from the words "....நோய் செய்யும் நூலோர் வளி முதலா எண்ணிய மூன்று".

    (4) The Medical Researchers of the Sangam period Tamil Nadu

    The historically known earliest Medical Researchers of Tamil Nadu of the Sangam periods of both Second Tamil Sangam (B.C.650-B.C.325) and third Tamil Sangam (B.C.325-A.D.350) were Sage/Poet Agastiyar and Sage/Poet Theraiyar.

    (5) Medical Treatises of Sangam period Tamil Nadu

    The earliest known Medical Practitioner who performed indepth study in Tamil Medicines and developed same was Sage Agastiyar, who was historically known to have lived in Pothikaimalai in present Thiruneveli of Tamil Nadu during the early part of Second Thamil Sangam period (B.C.650-325) at the time the Ramayana episode took place in A.D.550. He composed a treatise named "Aayul Vetham", the practice of which continued to be prevalent during the Third Sangam Period (B.C.325-A.D.350)

    In Kerala (Tamil Chera kingdom) of the period A.D.175 the great Tamil Epic Silappathikarem composed by Poet Ilango Adikal the younger brother of the great Tamil Chera king Chenguttuvan while describing the captal city of Cholas specifically refers to the quarters of Medical Practioners and Astrologers the streets of Kaviripoompattinam. He refers to same as "....Aayul Vetharum, Kaalakkanitharum....." Hence it is seen the Sage Agastiyar's Medical Treatise named "Aayul Vedam" composed by Sage Agastiyar was practiced in Tamil Nadu even during the period of the great Tamil Epic work Silappathikarem.

    He also composed a treatise in Tamil named "Vakadam" based on the Vakadam composed by the Sage Nandi, "incorporating much of the earlier Tamil Medical knowledge of Tamil Nadu", and the usefull informations from the other Ayurveda treatises. This treatise fortunately is available today more or less in full but with the name given subsequently as "Agastiyar 2000" (printed in three parts). The origin of this "Tamil Medical Treatise" by Agastiyar and it's contents are clearly given in the "Kaapu" and "Avaiyaddakkam" of same.

    ஆயுருவேதஞ் சொன்ன அதன்படி வியாதியுள்ள
    நோய்க்குகுறி குணங்கள் கண்டு நுண்ணிய மருந்து செய்வாம்

    தீயவையறிந்து சூட்டுத் தெளிவினால் பரிகாரத்தால்
    மாயவே மறிந்து தானு மறுபடி வியாதியாமே.

    Sage/Poet "Theraiyar" a renowned Pandithan (in Medicine) has written his principal Mediacal Treatise named "Theraiyar Kaapiam", followed by Sikamani Nikandu, Sekarap-paa, Venpaa, Tharunaatakam, Pathanadai, Vannam for all of which the Theraiyar Kaapiam is the key Text. Theraiyar has lived in the city known as Thirumalaicheri (in the present Wallajah region) in Vellore in Tamil Nadu. The other Medical Treatises attributed to Theraiyar are - Therayar Vaithiyam 1001, Theraiyar Vaiththiya Kaaviyam 1500, Theraiyar Karisil.

    பாரினிற் சீர்பெறு பண்டிதன் மருவுந்து
    தேரன் காப்பியம், இது திறவு கோலாகுஞ்
    சிகாமணி நிகண்டு, சேகரப்பா, வெண்பா,
    தருநாடகம், பதநடை, வண்ணம்,
    அனைத்தெனுங் கதவுக்காம் இது திறக்க
    விதனை திண் மருத்துவர் தாம் அறிகுவரால்
    தீதறு திருமலைச்சேரி மாநகரில்
    வாழி நன் மருத்துவர் வாழி வாழியவே

    கருப்பொருள் வகையெனக் கருதுவது இவையெலாம்
    பரம்பரையினரும் பண்டிதவகையார்
    திறம்பெறு நிகண்டினிற் தெரிந்து கொள்ளுவார்
    இப்பொருணிலையெலாம் யான்நெடுத்த இயல்புறச்
    செப்பினான் காப்பியம் செப்பினான் முன்னரே.


    Important Note

    Some Scholars have tried to mis-interprit the word "Seppinaan" as Telugu word for "he said" and hence Theraiyar was a Telugu, and hence the Theraiyar Kaapiam could be dated to the Naayakkar period which is totally wrong. During the Sangam period Tamil Karikal Cholan captured and brought the regions below Krishna river of present Andhra Pradesh under his direct rule and appointed members from his Chola royal family as his viceroys in these regions who subsequently were known as Telugu Chodas (Cholas). Most of the kings of these regions of the medieval period proudly called themselves as the decendents of the Karikat Cholan in their copper plates and Inscriptions.

    "Sepputhal" meaning "to say" in Tamil was a Sangam period Tamil Word found in the Tamil Literature of that period which was subsequently also used in the regions ruled by Telugu Chodas. However the use of this word gradually lost it's ground in Tamil Nadu over a period, but remained in use in Telungu Choda regions of Andhra Pradesh. Hence based on this word found in Theraiyar Kaapiam it is incorrect to date the age of this Medical Treatise of Theraiyar as of latter (Nayakkar) period but of Sangam Period.

    Further Theraiyar Kaapiam refers to one among the Dance forms performed by Mathavi the second Heroine in the Kerala Tamil Epic work of the Sangam Period Tamil Nadu the "Silappathikaram" was the "...தேர் முன் நின்ற திசைமுகன் காணப் பாரதி யாடிய வியன் "பாண்டரங்கமும்"....". The same Dance is made reference in the Theraiyar's Kaapiam as "...பாண்டரங்கன் கடக்கடல் பவளவண்ணம் பெரும் தாண்டவம் புரியுமித் தாணு லோகத்தில்.....which was peculier only to Tamil Nadu, and not even mentioned in the Sanskrit "Naatiya Sastra of Baratha Muni". Hence it goes to prove that the Theraiyar Kaapiam belonged to the Sangam Period and not to the Naayakkar Period in Tamil Nadu.

    Further it should be noted Theraiyar Kaapiam starts with a verse on "Guru" followed by "Guru Varalaaru". It could be clearly seen the former is a latter period Tamil with the phrase "Periyasamy Thirumoolar arul". Samy is a term more Telugu or late period coloquial Tamil which definitely an addition by the transcriber of this text from Old Olaa-Leaves to New Olaa-Leaves, while the Tamil in the Guru Varalaaru is more akin to Sangam period Tamil. This too could have made these Scholars to conclude that the Treatise was of a Telugu origin and hence Theraiyar was a Telugu which is absolutely false.







    Further these Medical Practitioners of this period were also aware of a system of Medicines which were being used for the relevent deceases as appropriate. From a reference in Sangam Period Poetic work "Iynththinai Iympathu" we note strong medicines were used in the healing of wounds.





    Pallava Period

    Chola Period

    During the Chola period we note there had been many Medical Centres and Hospitals in various regions in Chola Nadu.

    During the period of Rajendra Chola - 1 (A.D.1012-1044) there had been a free Medical Centre at Pandaravaadai in Thanjavur district founded in the year A.D.1015 by Kundavai Piraatiyaar the elder sister of Rajaraja Chola - 1 and the aunt of Rajendra Chola, and the Savarnan (= Physician) in charge of this Medical Centre was Savarnan - Araiyan Mathuranthahan.

    There had been another Medical Centre at Thiruvaduthurai in Thanjavur which too was founded by Paranthahan Kundavai Piraatiyaar in the year A.D.1016 which remained in charge of the Physician Savarnan - Araiyan Chandrasekhar alias Uttama Chola Acalan and his decendents as the Physicians of this region and were conferred with houses and lands for it's running and maintenance.

    Kundavai Pirattiyaar also built a Hospital at Pandaravaadai in Thanjavur in the name of her father as Sundara Cholan Vinnagar Aathular Saalai (= Hospital) and provided Medical Funds (Vaidya Bokam) to run same, and land to build a house to Savarnan - Araiyan Mathuranthahan and his decendents who were natives of the village Marukal. Kundavai Piraatiyaar gave a further grant in the year A.D.1019 to meet the shortfall of funds required to mainitain this Medical Centre.

    During the period of Virarajendra Chola the third son of Rajendra Chola - 1 there had been a Vedic/Agamic college instituted with hostal facilities for these students to reside and study at the Mathuraanthaha Chathurvedimangalam (the present Mathuraanthakam of the Kanchipuram district) which was formed during this period. There had been an internal 'Aathular Saalai' (hospital) named as "Vira Cholan" after Virarajendra Chola, to look after the health needs from various diseases of the teachers and the students of this college. This hospital was in charge of a Physician named as Kodhandaraman Aswathaaman Bhattan, and had a Salliyaikirikai Pannuvaan (Surgeon), and Female Nurses to to prepare the medicine and food according to the type of illness of the Patients. This internal hospital had facilities of fifteen beds and a store room within with the medicinal herbs required for a whole year was stocked.

    During the period of rule of Vikkrama Chola a native named Thevur Udaiyaan Velaan Manatukkiniyan alias Viratarajan established at Thiruppukalur in the present Thanjavur district a Hospital and Matha on the north bank of Mudikondasola peraru for the benefit of the sick and destitue (Athularaiyum Anatharaiyum). The Sabai sold some land as Athula Salaippura Iraiyili after receiving the cost from the treasury of the temple.

    During the period of Rajaraja Chola - 2 he built a new Temple to God Siva near Aduthurai in Kumbakonam disctrict which was too named as Rajarajaeswarem (re-named as Ayeratheswarem during Nayakkar period) at Rajarajapuram (re-named as Darasuram during the Nayakkar period). Adjacent to this temple there had been a settlement of Medical Practitioners and it came to be as Maruthuvakudi. The subsequent extention of this region of Medical Practitioners was known as Melai Maruthuvakudi.
